How to use your record

Use the tracker for short periods when it feels useful

This tracker can help you keep recent observations together before an appointment or during another period you want to remember.

Complete only the fields that feel useful. You do not need to track every symptom every day, and you can stop using the tracker if daily monitoring feels stressful.

Symptoms can have many possible causes. This tracker records your observations; it cannot tell whether a symptom is caused by hypothyroidism or another condition.
